How to Order a Test

Aviir’s elite Client Service Professionals are available to help you get started. We can be reached at 1-888-287-4217 Monday through Friday from 8am to 5pm Pacific Time.

Follow these steps to get started with Aviir:

  1. Contact Client Services by phone at 1-888-287-4217 (press 1), email at clientservices@aviir.com or click Contact Us. A representative will create an account for you.
  2. After your account is created, a startup kit including requisitions, supplies and a Directory of Service will be supplied so you can begin to request tests.
  3. The Cardiometabolic tests are covered by most insurance companies, and Medicare patients are not likely to have any out of pocket cost.

Specimen Collection Options

  1. If the patient chooses Aviir's Concierge Phlebotomy Service, a courteous and professional phlebotomist will be dispatched to collect the specimen at a convenient place and time. A nominal fee of $15.00 will be charged to the patient for this service.
  2. Specimens can be sent directly from your office to Aviir's laboratory facility. Client Services will work with you to set up specimen submission logistics.

Required Information for Ordering Tests

Requisitions will be provided to you for ordering tests from Aviir. Please make sure to fill out all required areas including:

  1. Patient Information
  2. Patient Insurance Information - please have the patient read and sign the assignment of insurance benefits
  3. Specimen Information
  4. If requesting that a Framingham Risk Score is to be calculated, fill in the necessary clinical information
  5. Select and fill in the appropriate ICD-9 Code

Test Results

Results are sent to the physician either by fax and/or mail. If required, we would be happy to provide more than one copy of the test report and/or copies to additional physicians.
